Almost all maltreatment complaints regarding Hockey Canada participants are submitted directly to Hockey Canada's ITP.  However, a few provinces have set up their own ITP and complaints go directly to them.  From the list below, please  select the province/territory where the person you are reporting is located.  You will be directed to the appropriate complaint form.
